Send us your patient's sleep study and we'll take it from there. A simple, three-step process to get their oral appliance therapy started.
GETTING STARTED
Required Documents
and Prescription
Thank you for your referral. To initiate sleep apnea treatment, the following documentation and prescriptions are required from your office.
Once we have everything below, our team will reach out directly to the patient to schedule their consultation.

Diagnostic Sleep Study
Fax a copy of your patient's full sleep study report to our office at 413-729-3646.

Prescription Form
Send a completed prescription form or letter of medical necessity (PDF).

Insurance Information
We will bill Medicare and most insurance providers for you. Call us and we'll check coverage on the patient's behalf.
